Documents and Forms
A School Entrance Health Form is required for all NEW students enrolling at Little Flock Preschool and Good Shepherd Christian Academy. State law requires that each child is immunized and receives a comprehensive physical examination before entering school. Students will not be allowed to start school until the School Entrance Health Form is submitted. Returning (re-enrolled) students entering Kindergarten (K5) and 6th Grade are requested to provide the school an updated vaccination record to show they meet age appropriate & dosage state requirements.
